Currently I scan a hand sketch then touch up (if needed w/ a 9x12 wacom), colorize, or paint over underlay - all dependant on project req's.
Will soon be utilizing a Cintiq 18 (I'm told via management) for sketch generation - they were so impressed by what could be turned out and how fast while I demo'd the sample product for them. Very cool piece of equipment.

Side note: When our local wacom reseller found out we were interested in them, they were very generous and let us (3 designers) demo an 18sx Cintiq for a week - great way to try it out and get hooked. can be addictive.
